Lost/Stolen Passport


1. Anyone whose previous passport was lost or stolen must apply in person.

2. To qualify for a replacement passport, you must provide evidence of your citizenship and identity. Examples of acceptable proof of identity or citizenship include:

  • An original expired U.S. Passport.
  • An original U.S. Certificate of Naturalization
  • An original U.S. driver's license (proof of identity only)
  • An original U.S. birth certificate (proof of citizenship only)


An officer may consider any other evidence of citizenship or identity that the applicant might provide, but can issue a new passport only if convinced of the applicant's identity and U.S. citizenship.


3. Statement Regarding a Lost or Stolen Passport (Form DS-64) and an incident report from the police in the jurisdiction where the loss occurred. If the person who lost the passport is a minor under 16 years of age, the minor must come to the consulate with parents or legal guardians. The parents or legal guardians must bring their passports. If the parents or legal guardians have not accompanied the child, then the person in loco parentis must present his/her passport and a notarized statement from the parents or legal guardians authorizing the person to make the application. 4. Two (2) identical photographs taken within the last six months.


Photograph dimensions: Photographs must be taken within the last 6 months and measure 50 mm by 50 mm in size. Pictures must be clear, front view, full face, taken in normal street attire without a hat or dark glasses, and printed on thin paper with a plain light (white or off-white) background. The photos may be in color or black and white. Digitized photos must meet the previously stated qualifications and will be accepted for use at the discretion of Passport Services.

  • Properly completed Form DS-11, Application for a U.S. Passport (blue or black ink only)
  • Proper fees in cash (U.S. dollars or Rupee equivalent;
  • Replacement of lost passport, 16 years old and over: $100.00
  • Replacement of lost passport, under 16: $85.00
