Diversity Visas

Applicants for Diversity Visas require the following documents:

Application (Form DS230, parts I and II)

Supplemental Registration (Form DSP-122)

Birth Certificates (if unobtainable, click here)

Police Clearance Certificates

Court and Prison Records, if applicable

Evidence that you have a high school diploma or equivalent

Passport

Proof of Relationship with your spouse and children (if applicable)

Visa Photographs
